Youth Hockey Training Waiver & Release of Liability

By registering for and participating in any private lesson, small group session, clinic, camp, or training program (“Programs”) offered by Ben Stadey and Ben Stadey Hockey Training (“Coach/Trainer”), I acknowledge and agree to the following terms on behalf of myself and my child/participant:

Assumption of Risk
I understand that participation in hockey training and athletic activities involves inherent risks, including but not limited to: falls, collisions, and contact with other participants; injuries caused by pucks, sticks, boards, nets, or ice surfaces; muscle strains, sprains, fractures, concussions, and other physical injuries; and risks associated with physical exertion and athletic conditioning. I voluntarily assume all risks associated with participation in these Programs.

Medical Fitness
I certify that the participant is physically fit and able to participate in hockey-related activities. I understand it is my responsibility to consult with a physician regarding any medical concerns prior to participation. I agree to disclose any medical conditions, allergies, injuries, or limitations that may affect participation.

Release of Liability
To the fullest extent permitted by law, I release, waive, and discharge Ben Stadey, Ben Stadey Hockey Training, and their owners, coaches, trainers, employees, contractors, volunteers, affiliates, and facility operators from any and all claims, liabilities, damages, demands, costs, or causes of action arising out of or related to participation in the Programs, including injury, illness, property damage, or loss, except in cases of gross negligence or willful misconduct.

Emergency Medical Treatment
In the event of an emergency, I authorize Ben Stadey, Ben Stadey Hockey Training, and their representatives to obtain emergency medical treatment for the participant if I cannot be reached immediately. I understand that I am financially responsible for any medical expenses incurred.

Equipment Responsibility
I understand that properly fitted hockey equipment is required for participation unless otherwise specified by the coach. I accept responsibility for ensuring the participant uses safe and appropriate equipment.

Conduct & Safety
Participants are expected to behave respectfully and follow all coach instructions and facility rules. Ben Stadey and Ben Stadey Hockey Training reserve the right to remove any participant from a session for unsafe, disruptive, or inappropriate behavior without refund.

Cancellation & Refund Policy
Sessions may be subject to cancellation or rescheduling due to weather, facility issues, illness, emergencies, or scheduling conflicts. Refunds, credits, and rescheduling policies are determined solely by Ben Stadey and Ben Stadey Hockey Training. By booking, I acknowledge and accept the posted cancellation policy.

Photo & Video Release
I grant permission for photographs and videos taken during training sessions to be used by Ben Stadey and Ben Stadey Hockey Training for promotional, educational, and social media purposes unless I submit a written opt-out request prior to participation.

Acknowledgment of Agreement
By checking the acknowledgment box during registration and proceeding with booking/payment, I confirm that: I am the parent/legal guardian of the participant, or I am 18 years of age or older; I have read and understand this waiver and release; I voluntarily agree to these terms; and I understand that this agreement is legally binding.
